Rates
$190 for initial intake if behavior rating scales are not needed.
$385 for initial diagnostic evaluation for psychologists, including behavior rating scales if utilized as a part of the evaluation.
$150 for initial diagnostic evaluation for licensed social workers and licensed practical counselors
$150 for each court mediation session
$150 for each session following the initial evaluation for psychologists
$100 for each session following the initial session for licensed social workers and licensed practical counselors
$150 for each hour of psychological testing.
$550 for SPARK testing
$519 for Gastric evaluation
$2500 for Psycho-educational Evaluation. Includes 8 1/2 hours of testing,
There may be charges for other services, such as testing, court appearances or depositions, reports, phone calls exceeding 10 minutes with you or third parties (attorneys, doctors, insurance carriers, managed care reviewers). Payment is expected at the time of service.
Insurance
The charges for our services are covered to varying extents by most health insurance polices. Our billing office is happy to assist you in filing with your insurance carrier for reimbursement. However, our contract for payment is with the client and not with the client’s insurance carrier. We encourage you to contact your insurance carrier to assure benefits coverage.
Insurance coverage varies a great deal among different policies. We will be happy to contact your insurance carrier for you and verify your coverage. The basic questions to be answered include:
- Is there a deductible to be met?
2.Are outpatient mental health benefits covered and is pre-certification necessary?
3. What percentage of the fee will the insurance cover?
4. Is there a limit on the number of sessions allowed?
5. Are reviews by managed care required?
6. If there is more than one insurance company, are there special rules to follow?
Clients must make full payment at the time service is rendered until the deductible has been met . Thereafter, clients must pay their co-payment amount, and we will submit claims to the insurance company for the remaining amount. We make every effort to determine your benefits, however, it is the client and not the insurance company who is ultimately responsible for payment.
Insurance companies require that a diagnosis be provided in order for a claim to be processed. In some cases, insurance companies will not pay for certain diagnoses. If you have any concerns about your insurance billing, please discuss them with Jessica, our Insurance Manager.

Cash, check and most major credit cards accepted for payment.
Cancellation Policy
It is very important that the office is given at least 24 hours notice when you are unable to attend a session. You will be charged $50 for any missed appointment with a licensed social worker or licensed practical counselor or $75 for any missed appointment with a psychologist. This fee is not reimbursable through your insurance benefits.
